Your Library Card
Lakewood Public Library, consisting of the Main Library and the Madison Branch, is an independent system with its own unique Library card and policies. Registration is free and open to every resident of the state of Ohio.
Every Library card comes with a Personal Identification Number (PIN) that can be used to renew books, place holds, check the status of materials online, and access time on Library computers.
Patrons are asked to notify the Library immediately when their card is lost or stolen, their PIN is compromised or they have changed their address or phone number.
Registering for an Adult Library Card
Adult applicants may visit the Main Library or Madison Branch to complete a library card application and present a valid photo ID along with proof of current address. Proof of current address includes the following:
- A recent (less than 30 days old) utility bill
- A recent bank statement
- A current rent receipt or lease agreement
- Government issued ID or statement
Adult applicants may also complete the form below and then visit the Main Library or Madison Branch and present a valid photo ID along with proof of current address to pick up their library card.
Registering for a Juvenile Library Card
Children ages two years to twelve years may register for a juvenile library card on their own using this form.
Borrowing
All circulating materials are available for a two-week loan period. A total of fifty items may be checked out on an adult card and a total of twenty items may be checked out on a juvenile card.
Renewal
Most materials may be renewed up to five times, unless the item is on hold for another patron. Periodicals and eBooks are excluded. Items may be renewed in person, through the online catalog, or by calling the twenty-four hour renewal line at (216) 226-8275, ext. 150.
Late Fees
The overdue fee for most items is ten cents per day. An overdue DVD will result in a fee of fifty cents per day. Borrowing privileges will be suspended when accumulated fees reach $10.00. After thirty days, any item that is still overdue will be billed to the patron’s account for the replacement cost until the item is returned.
Replacement Card
Damaged cards can be exchanged for free. However, there is a $2.00 fee for replacement of a lost card. To replace a lost card, patrons must present a valid photo ID, all overdue items must be returned or renewed, and all outstanding fees must be paid in full in order to obtain a replacement card.
