Adult & Electronic Services » Request for Proctoring Services Form
Students can have written and electronic tests proctored at Lakewood Public Library Main Branch. This service is free.
How to begin
The student or testing institution must complete a form for each course requiring proctoring services at Lakewood Public Library. In addition to completing this form, be sure to read and comply with all proctoring requirements of your school, college, or university.
Proctoring is available only to patrons with a Lakewood Public Library card in good standing. If you do not have a Lakewood Public Library card, you will need to register for one prior to test day. For questions about registering for a card, please call the Technology Center at (216) 226-8275, ext. 127.
Please note that all requests must be filled out completely. Requests that are not filled out completely will not be processed. Again, a form must be submitted for each course requiring proctoring services. Questions? Contact us by phone (216) 226-8275 ext. 127 or by email at proctoring@lkwdpl.org.
Thank you.
